The Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) includes more than 300 health-care providers and organizations working together to create a better health-care system to improve health outcomes and provide better health-care experiences for everyone in our communities.
 
First, we need to hear from you about your health-care experiences. Your answers will help the FLA OHT to understand how to build a better health-care system that addresses the mental, physical, emotional, spiritual and social health and wellness needs in our communities. 
 
This survey is anonymous and should take about 10 minutes to complete. It is open to any resident of Frontenac, Lennox and Addington counties, including Gananoque, Lansdowne and all the communities in the FLA region. The survey is also open to non-residents who are caregivers of Frontenac, Lennox and Addington residents.
