Sleep Tracking Survey

Informed Consent

Dear Participant,

We are asking you to complete an anonymous survey as part of a research study, titled “Survey of Older Adult Use of Sleep Tracking Devices.” The Cedars-Sinai Principal Investigator is Dr. Lisa Gualtieri. The study contact phone number at Cedars-Sinai is 310-657-4337.

Taking part in this research study is voluntary. You have the right to choose not to take part. You can also stop taking part at any time. You will not lose any services, benefits or rights you would normally have if you choose not to take part or stop taking part. Your choice not to take part or to stop taking part will not affect the care you get at Cedars-Sinai.

We are doing this study to help us learn how devices that track sleep affect the bedtime and morning routines of older adults. The survey will include questions to find out your experiences with sleep-tracking devices. We think this one-time survey should take about 7 minutes for you to complete. Some questions may make you feel uncomfortable or embarrassed. The survey is anonymous. It will have no information that could be used to identify you.

We expect to get survey responses from about 100 people.

You should not expect to benefit from taking part in this research study. We hope the information learned from this research study will help to increase understanding of how sleep tracking affects older adults.

You will not be paid for taking part in this research study.

At the end of the survey, you may enter a raffle for 1 of 3 $50 Amazon gift cards. This is voluntary. You will be provided a separate link to provide your identifiable information to receive payment. This identifiable information will not be linked with your specific survey response. We will randomly select the winners 1 month after the survey closes for everyone.

If you choose not to participate in survey, you may still enter voluntary raffle. A separate link to raffle will be provided as described above.

The researchers are conducting an opportunity drawing with the chance to 1 of 3 $50 Amazon gift cards. You have about a 1 in 34 chance of winning the Drawing. No individual is guaranteed to win any prize. There is no other compensation for taking part in the research study, other than a Drawing entry. All Drawing entries have an equal chance of winning.

Research Participant Entry: This is voluntary and will be collected separately. Following conclusion of survey, you will be asked if you would like to participate in raffle. If you choose to participate, you will be provided a separate link to provide your identifiable information to receive payment. This identifiable information will not be linked with your specific survey response.

The winner of the Drawing will be randomly selected 1 month after the survey closes for everyone. If you win, you will be contacted by email.
