Before completing this application form, please review the 2026 Election Guide and Director Profile which provides detailed information about the Board of Directors and the screening and election process.

To be considered, you must submit a completed application that includes:

  • Three reference letters (from your peers or supervisors)
  • Your current CV/resume (in Word or PDF format)
  • A candidate statement of 300 words or less indicating why you wish to be considered for a position on the Board of Directors.

You may wish to gather these before beginning the survey, which will take approximately 30-45 minutes to complete.

Please consider drafting your responses in a Word document saved to your personal computer until your application is submitted.

DO NOT refresh your browser window as your survey entries will be lost, and you will be required to begin again.

Although we encourage you to complete your application in one sitting, if you do not clear your cache you can return to the application and complete it later within the same browser.
