EPCC Exit/Past Patient Survey

Thank you for your participation in this survey, your feedback in important. The purpose of this survey is to better understand the needs and experience of past pregnancy care patients of Encompass Pregnancy Care Cranbrook (EPCC). Results will be used to support the EPCC, and guide service delivery to improve the experiences of pregnancy care patients.

This survey is anonymous and any identifiable information will be removed, and your identity will remain completely confidential. Your participation in this survey is completely voluntary. We encourage your response but do not require you to respond.

There are open-ended questions in this survey. We do not wish to collect any information that might identify you or another person. We encourage you not to provide any identifying information, such as names, in your answers to open ended questions.
