This survey is for families of all children and young people aged 0 to 25 years with special educational needs and/or a disability (SEND), not just those with an Education, Health and Care Plan (EHCP) or on formal Special Educational Needs (SEN) support at school.
Please fill out the survey. This will help us understand your experience over the last 12 months. Let us know what is working well and what could work better. We will share the results with providers of SEND services and Ofsted. This will ensure that services take your views into account when providing SEND services in Derbyshire. A real chance for you to have your say!
SEND services include any provider of help, advice, guidance, and support for children and young people aged 0 to 25 years with special educational needs and disabilities. This could be an educational setting, a health care provider, social care, a local charity, a community group, or an individual.
Survey answers are anonymous so please do not include any names in your answers. If you care for more than one child with any kind of SEND, you can complete a separate survey for each child. Please note you may need to clear your browser history on a phone or tablet in order to complete a second or subsequent time.
The survey should take approximately 20 - 30 minutes to complete.
