Collecting your views on the provision of healthcare services in Bristol, N Somerset and South Glos

We would like to hear your experiences with local health services for your child or young person with SEND (up to the age of 25).

Please answer based on your current or recent (within the last two years) experiences of support.
We welcome responses from parents and carers of children or young people with any additional need or disability - diagnosed or not - including: if your child is neurodivergent (autistic, ADHD, etc); has learning disabilities; communication difficulties; sensory needs (hearing or vision); physical disability; complex health needs; social, emotional and mental health difficulties.

We are interested in how well medical services and related service providers understand and support children and young people with special educational needs and disabilities. This could include hospitals and GPs, but also other services such as dentists, health visitors and school nurses, assessment services and more. We also want to know how well other professionals and service providers (such as schools or social care) work with medical services to ensure joined-up care for your families.

Your views help us represent all parent carer families across the region.

This survey takes about twenty minutes to complete. Please fill in one survey per child or young person with additional needs.
