Survey for Care Providers of Day Opportunities
Shropshire Council is working to gather feedback relating to Day Opportunities in order to inform future commissioning and learn more about the needs of adults with learning disabilities who use services, their carers and the professionals working to deliver, assess and arrange support.

This survey is designed for the Providers of day opportunities. It focuses on those services delivered by external providers and not those delivered by Shropshire Council.

Further surveys are available for individuals and their carers/ family members, and also the social care workforce.
