DDO Extra Life 2025 Post-Donation Survey

We want to thank you for your donation by getting you some codes that can be redeemed through the DDO Store for items from the Community Loot List!

Make your selections and get me any additional information you need below. Make sure to provide me with a valid email address so that I can most easily get you your codes. If you cannot provide me with a valid email address, a DDO Forum Display Name is the next best option, followed by your Official Dungeons & Dragons Online Discord handle. If you don't yet know what you want, NO HURRY, fill out this survey once you know what you want. Please get your information to me by December 31st, 2025. If you wish to be entered into the Group Incentive giveaways I will need your survey sent in before December 1st, 2025. Thanks!

Please provide me with your email address so that I can send you your codes. If you cannot provide an email address, your DDO Forum Display Name or Official Dungeons & Dragons Online Discord handle is also acceptable:(Required.)
2.Please let me know how much you donated. Feel free to include any processing fees associated with your donation.
3.Here is how much you can pick:

$10: 1 pick
$20: 2 additional picks (3 total)
$30: 1 additional pick (4 total)
$40: 2 additional picks (6 total)
$60: 4 additional picks (10 total)

For donations above $60, your additional picks will align to how much above you donated. So, if you donated $100 you would get 16 picks.

These will be distributed to you through codes that are entered through Redeem Code in the DDO Store inside of the game. You are free to distribute these codes to friends or family as you wish.

If you wish to receive multiple of the same item, indicate that in your survey message to me.

Donations that have already been made to Team Standing Stone Games in 2025 are eligible for these incentives! Simply fill out the survey and let us know when you donated and how much, along with your picks.

Donations to any member of Team Standing Stone Games counts.

If you aren't sure what you want at the time you make the donation feel free to wait until you are ready to send in your survey. Just make sure to get it to us before the end of December!

The Customer Service team does not have the ability to assist with Extra Life code concerns, although they are good about forwarding these tickets to me when necessary. However, the easiest and fastest way to get a code question or issue resolved is to contact me (Cordovan) through the DDO Forums or Discord. Thank you and have fun!
GROUP INCENTIVE INFORMATION:

Once we reach each fundraising threshold additional incentives will be unlocked:

$10,000 – Points! We'll select 50 people who donate at least ten dollars to the fundraising effort through a random-number-generated giveaway to receive a 100 DDO Point code.
$20,000 – More Points! We'll select an additional 25 people who donate at least ten dollars to the fundraising effort through a random-number-generated giveaway to receive a 500 DDO Point code.
$30,000 – Points and an expansion giveaway! We'll select an additional 25 people who donate at least ten dollars to the fundraising effort through a random-number-generated giveaway to receive a 500 DDO Point code. We'll also give away an Ultimate Edition of The Chill of Ravenloft to someone who has donated at least $10!

All group incentive giveaways will take place after November 30th, and winners will be notified through the contact information provided in question 1. You must fill out this survey before December 1st in order to qualify for the group incentive giveaways since I use this list for the giveaway. Thank you!
