The College is in discussions with the Government of Ontario regarding the current authority of RPs to sign off paperwork related to service provision. CRPO is proposing an expansion on current limitations to provide better access for clients and to ensure continuity of care.
The results of this survey will be presented to the Government and used to assist in building a case for expanded signing authority.
This survey will ask about several topics:
- Gender affirmation* genital surgery
- Capacity assessments
- Ontario Disability Support Program Employment Supports: Verification of Disability / Impairment Forms
- Ontario Disability Support Program Disability Determination Package - Activities of Daily Living (ADL) Form
- OCF-18 forms
- General practices related to forms and fees
These forms will be explained in further detail later in the survey.

Comments submitted may be used and distributed in a summary report. All comments are anonymous and will be used to compliment relevant and associated data points.
