1.SURVEY INFORMED CONSENT
You are being invited to participate in a research study conducted by Caylie Peet, a doctoral candidate in Community Engagement at Point Park University.
The purpose of this study is to better understand how placemaking and walkability initiatives influence community life and local economic activity in Grand Rapids, Michigan.

If you agree to participate, you will complete an online survey about your experiences, perceptions, and observations related to walkability, placemaking, neighborhood engagement, and local business activity in Grand Rapids. The survey should take approximately 10–15 minutes to complete.

Your participation is voluntary. You may choose not to participate, skip any question you do not wish to answer, or stop taking the survey at any time without penalty.

This study involves minimal risk. Some questions may ask you to reflect on your experiences in your neighborhood, community, or local business environment, which may feel mildly uncomfortable. There are no direct benefits to participating in this study. However, your responses may help contribute to research that informs future community planning, placemaking, accessibility, and local economic development efforts in mid-sized cities.
