Please fill out the form below and choose your shift for our 2025 Sock Sorting & Distribution Event.

All volunteers must sign-up individually.

Date: Saturday, February 1st, 2025. Choose from two shifts (9:00 am to 11:30 am or 12:30 to 3:00 pm)

Where: Poly Five Plastics - 205 Limestone Cres., North York, Ontario (Steeles and Keele)

Things to note:
- We will be working in an unheated factory, so please dress warmly (it will be quite cold).
- Please use caution when walking in the parking lot as it may be slippery.
- You must be 12 years of age or older to participate
- Volunteers between 12 and 18 must have a parent/guardian register on their behalf.
- High school students can earn volunteer hours.

All volunteers will receive an event day information email prior to Saturday, February 1st.

* 8. WAIVER AND RELEASE RE: EVENT PARTICIPATION

I acknowledge and agree that I will be a volunteer participant and accept and take responsibility for my welfare and safety during the event scheduled to be held at Poly 5 Plastics, 205 Limestone Cres, Toronto Ontario on February 1, 2025 (the “Event”).

I hereby assume all of the risks of participating and/or volunteering in
the Event including, but not limited to, any risks that may arise from negligence or carelessness on the part of the person or entities being released, from dangerous or effective equipment or property owned, maintained, controlled or used by them, or because of their possible liability without fault.

I hereby unconditionally release, waive, discharge and covenant not to sue Socks 4 Souls Canada, the officers, directors, employees, volunteers, agents,
or administrators of Socks 4 Souls Canada, or any sponsoring agencies and sponsors (hereinafter referred to as the “Releasees”) from any and all liability, responsibility, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by myself or my child(ren), heirs, next of kin, and any other person, whether caused by the direct or indirect negligence of the Releasees, or otherwise, as a result of my participation in the Event.

I acknowledge that:

1. I am at least 18 years of age and competent to sign this Waiver and Release or that I have otherwise obtained the consent of my parent or legal guardian;
2. I have read this Waiver and Release thoroughly and understands the contents of such Waiver and Release;
3. I am voluntarily executing this Waiver and Release;
4. I am fully aware of the potential risks involved and the potential hazards
connected with participation in the Event;
5. I voluntarily assume full responsibility for these acknowledged risks and
hazards;
6. I agree to indemnify and hold harmless the Releasees from any loss, liability, damage or costs, including court costs and attorney fees that they may incur as a result of my participation in the Event, whether caused by the direct or indirect negligence of the Releasees, or otherwise; and
7. It is my express intent that this Waiver and Release shall bind the members of my family and spouse, heirs, assigns, and personal representative.

* 9. CONSENT TO INTERVIEW, PHOTOGRAPH, VIDEO AND RELEASE OF
INFORMATION

I authorize Socks 4 Souls Canada and their media affiliates (including but not limited to print, radio, television, and social media) to publish and/or broadcast in connection with any product or service for any purpose (including, promotion, publication, advertising, broadcast and/or trade) in all markets, media or technology now known or hereafter developed. I consent to the use of the resulting material in perpetuity, without payment or compensation by Socks 4 Souls Canada in any form or translation including radio broadcasts, newspapers and other publications, and other media including television, Internet or film releases if so required. I acknowledge that by signing this document I shall fully discharge and release Socks 4 Souls Canada and/or the Producer and their representatives, respectively from any and all claims, demands, damages actions or causes of actions whatsoever, which I, my heirs, executors, administrators or next of kin may now or hereafter have against them or any part of them, including but not limited to, any claims arising from publication of the materials. I acknowledge on behalf of myself or my child by providing this Consent, and such consent cannot be revoked.
