Contact is a charity that provides information, advice and support to families across the UK. We support families with disabled children and children with additional support needs, or who have health and rare conditions, from 0 to 25 years old. Families do not need a diagnosis or an assessment for their child to get support from Contact.
Contact also provides opportunities for parents to volunteer and campaign to ensure their voices, concerns and challenges are heard by politicians and decision makers.
We are carrying out this survey to find out more about the information and support needs and parent voices in decision making and policy, so we can improve what we do as a charity for families with disabled children and children with additional needs.
This survey should only take five to ten minutes to complete, and almost all the questions are tick-box
The survey is anonymous as you are not required to provide your name or any identifying information.
