About this questionnaire

Contact is a national charity who supports families of children with additional needs.

We want to understand the experiences of parents of young children (aged 0–5) in England who may need extra support, have learning differences, or whose development is raising questions. Your child does not need a diagnosis or formal label for you to take part.

All data is anonymised. We will not ask you for any personal details unless you wish to provide your contact details at the end of the questionnaire. We may use the words you share in quotations to explain survey findings, but we will never share personal data. We will use the results of this survey to develop Contact’s services and to influence government policy, including the Government’s SEND White Paper consultation (details and links are shared at the end of this survey).
