The Colorado Disability Opportunity Office is studying the assets and needs of the Colorado disability community. This work will produce one of the country’s first statewide needs assessments for all disability types.
Please complete this survey if you have a disability or take care of someone who has a disability. The survey is voluntary and confidential. Your decision to participate will not affect you or your dependents’ insurance, health care, or relationship with any doctor, hospital, or state and federal agency. Survey responses will be aggregated, and no identifying information will be shared in reporting.
You must reside in Colorado and be at least 18 years old to take the survey. Parents and guardians are welcome to complete this survey for or with their child.
It will take about 20 minutes to finish.
