The Shire of Collie is preparing a Public Health Plan as part of its obligations under the Public Health Act 2016. In doing this, the Shire is seeking community input into how public health can be improved or maintained within the Shire of Collie.
 
The Shire invites you to participate in this Community Public Health Survey, which will take roughly 5-10 minutes to complete. 

Completing the survey will help us better understand your needs and give us the evidence we need to advocate for better health outcomes for residents of the Shire of Collie both now and into the future.
 
You can complete the survey online, or request a printed copy from the Shire Office. The Closing Date for the Survey is Friday 31st August. Please return any printed responses to the Shire Office by that date.
