Thank you for participating in our survey! We are designing the One Mind Flourish@Work program to help build greater, more career-oriented job opportunities for people with Serious Mental Illness (SMI), with your input.
Your answers are completely anonymous and confidential, so please feel welcome to be as open as you feel comfortable. Some questions may ask about past experience- consider any of your past informal jobs, school work, and/or volunteer experiences when answering.
This survey is designed to take 10-15 minutes to complete, but you may take longer. Survey participation is not paid, but will benefit people with SMI by creating better job opportunities with employers. There is some risk of emotional disturbance, as questions may prompt reflection on unpleasant experiences. You may stop taking this survey at any time, without penalty.
