CSB Prevention Awards Nomination Survey

Introduction

Every year, CSB prevention staff across Virginia go above and beyond to uplift their communities through education, coalition-building, data-driven programming, and other essential work that often goes unrecognized. This year, we want to celebrate that work through peer nominations.

Who can nominate: Any CSB prevention staff member

Who can be nominated: Any CSB as a team or a specific CSB prevention staff member (self-nominations are welcome, but peer nominations are especially encouraged)
