Welcome to the 2017 Community Perception Survey for the cities of Brecksville, Broadview Heights, Independence and North Royalton.

We are working very hard to better understand youth health risk and protective behaviors in the community and across Ohio. As part of this effort, we are asking community members to participate in an anonymous survey to collect information about alcohol, marijuana, tobacco and prescription drug use among youth.  

The results will help guide prevention efforts in the community.  Your participation can help make a difference.

The survey is confidential and participation is voluntary. You can skip questions you do not want to answer and stop at any time. You must be an adult, 18 years or older, and live or work in one of the communities mentioned above to participate.

There are no risks to you for participating in this study, and you will not receive any payment for your participation.

After you have completed the survey, you will have an opportunity to enter a raffle for one of three $250 VISA gift cards. If you choose to enter the raffle, you will be redirected to a separate page where you will be asked to enter your name and contact information for the raffle. In this way, your answers to the survey will continue to remain totally anonymous.

The data from this survey is being analyzed by an independent consultant, Jeff M. Kretschmar, Ph.D. Dr Kretschmar is also a Research Assistant Professor and Managing Director at the Begun Center for Violence Prevention Research and Education. The Begun Center is part of the Jack, Joseph, and Morton Mandel School of Applied Social Sciences at Case Western Reserve University.
