At the end of the survey, the survey will calculate a total score to determine the health of your institution's culture of philanthropy, and provide some insights for areas of improvement.

If you are unable to answer a question, you may provide a comment in the "Other" field. This will impact your final score.

If you would like a copy of the assessment and a video recording  of our webinar "Raising Major Gifts and Elevating Your Culture of Philanthropy as the World Recovers from Crisis", presented at the AFP 2021 Conference, please provide your contact information at the conclusion.
