APM Volunteer Achievement Awards Survey

The APM Volunteer Achievement Awards recognise volunteering activity undertaken between 1 April 2025 and 31 March 2026 inclusive.

If you would like to recognise an individual or collective achievement within the APM volunteer community, please submit a nomination by completing the information requested in this form. Self-nominations are also accepted.
A nomination should be a summary of the nominee’s achievements and should be no longer than 500 words per criterion response

If you wish to submit nominations across multiple award categories, please complete a separate form for each nomination. The same survey link may be reused for multiple entries.

The closing date and time for nominations is midnight (12:00am UK / BST) on Sunday 5 April 2026.
