5.I certify that the nominee meets the following criteria and is eligible to be named an SIO Fellow:
  • SIO member has a history of at least five (5) consecutive years of SIO membership
  • SIO member holds/has held a senior position in the field of integrative oncology
  • SIO member demonstrates significant achievement in one or more of the following categories related to integrative oncology:
    • Clinical Practice
    • Education
    • Research
    • Administration
    • Advocacy
    • Service
  • Nominee agrees to attend the SIO conference at which the award will be announced. (Exceptions due to illness or other personal circumstances will be considered on a case by case basis.)
