Participation and Curricular Achievement 

Complete Achievement Form by chronicling your involvement, leadership and achievement in school, college, community activities or employment.  These achievements can include, but are not limited to fine arts, music, sports, acting, theatre, dance, writing, debate or involvement in social, religious, scientific, military, civic and leadership organizations.  Be sure to specify the number of years of involvement, as well as positions of leadership.  Please attach additional sheets of achievement and leadership detail if needed.

There has been increasing evidence that the design and condition of an educational facility has a significant impact on the quality of education and a student’s ability to learn. In 500 words or less, please describe how the layout, features, program spaces and general condition of your high school facility contributed to the opportunities and challenges of your education. Also, please describe potential changes to your high school facility that you would propose, which you feel if implemented, would improve the quality of education.
