What are the most important health and well-being issues in the communities you serve? The Bradford County Health Advisory Council and the Union County Health Advisory Council, in partnership with WellFlorida Council, the local health planning council, invite you to answer this Community Health Assessment survey. As a healthcare provider, social service (or related service) provider, and/or community partner, we need your input on the most pressing health-related issues facing Bradford County and Union County now and beyond 2026. The survey will be available from DATE through DATE, 2026. Community leaders will use your answers to take action towards a healthier community.
This survey has eight core questions along with some demographic items. It should take no more than 10 minutes to finish the survey. Your answers cannot be used to identify you. Please answer the survey only once.
To be eligible to take this survey:
· You must be at least 18 years old and
· Provide healthcare, social and related services to residents and/or be a partner to community health and well-being improvement efforts in Bradford and/or Union Counties.
