I permit the Boys & Girls Clubs of the Gulf Coast (BGCGC) and Molina Healthcare to photograph, film, or record my child during the Boys Conference event.

I understand that:
- Photos, videos, and audio of my child may be used to document and promote program activities.
- BGCGC and Molina Healthcare may use these images or recordings on social media, websites, printed materials, news/media outlets, and other promotional or educational materials.
- My child’s name or identifying information will not be shared publicly without my separate written consent.
- I may revoke this consent in writing at any time, but it will not apply to materials already created or released.

I release BGCGC and Molina Healthcare from any liability related to the use of these images or recordings.(Required.)
