ArtsUplift at the Cincinnati Art Museum: Prospective Participant Recruitment Form

Thank you for your interest in helping the Cincinnati Art Museum expand wellness programming for the cancer community through this ArtsUplift study.

Please complete the following pre-qualifying questions. Completion of this recruitment form does not guarantee participation. Selected participants will be notified via email after completion of this recruitment form to confirm participation.

If you qualify and are selected to participate in this study, you will be required to:

Select wellness programs from the designated program list
Register for programs
Attend programs and sign into attendance sheet
Receive a free family membership for the completion of 3 programs and the final survey
Receive a free family membership and a $50 stipend for completion of 4 programs and the final survey

The Cincinnati Art Museum may use quotes or other responses from the survey in reports and publications. Any personal information will be kept confidential. We respect your privacy. Our privacy policy ensures that all information remains confidential and will only be used for research purposes.

Your participation in the study is completely voluntary. You may withdraw at any time or refuse to participate entirely. There will be no penalty if such actions are taken except that you will forfeit your opportunity to attend specific activities at the museum for free (select programs) and forfeit your incentive.
