Introduction

Below you will find questions that should be completed to share your program/initiative with the human services field. Your responses will directly inform the article, so please be as specific as possible. None of the questions below are mandatory/required, so you can skip questions if they do not apply. The survey should take roughly 10 minutes to complete. CQL and NADSP will be selecting around 14 programs/initiatives total to share during National DSP Recognition Week. After reviewing the submissions, we will be in contact with you about the results of our selection process and potential next steps.
 
Before you begin your submission, please make sure you can answer “Yes” to the following:
  • Is this a specific program or initiative? (vs. a mission/vision statement)
  • Does your initiative/program have a long-lasting impact?
  • Has there been a positive effect on the lives/work of DSPs?
  • Are you able to provide DSP testimonials/quotes about that impact?
  • Could other organizations replicate your efforts?
