This Award Form is for Clubs of MORE THAN 40 MEMBERS. Please complete each question on this form. The submission deadline is September 18, 2026. Include your club's INTERNATIONAL SERVICE Projects for the period of July 1, 2025 through June 30, 2026.

2.CONCISELY list up to TEN (10) of your Club's International Service Projects, in the spaces provided. Examples: Project #1: Donation of Shelter Boxes. Project #2: Tuition for Mexico HS Student. Project #3: Purchased Stoves for Guatemala.
3.Give the AVERAGE percentage of your members involved in the TOTAL number of your projects.(Required.)
