2026 OTIS Homeschool Packs Signup Form

Please Note: One form must be completed per eligible child. Current age of child must be 6-17 years old. Limit one (1) Homeschool Pack per child. Parent or guardian must be a current member of OTIS FCU.

Limited number of Packs available. Pack contents may vary but will include a variety of back-to-school items, such as notebooks, pencils, glue sticks, and erasers. Packs will be reserved on a first come, first served basis. Deadline to reserve your Pack(s) is Friday, September 4, 2026 at 4:00pm.

IMPORTANT INFORMATION ABOUT PACK PICKUP: Within 72 hours of submitting your form, you will receive an email with either 1) an individual pickup code and additional pickup information, or 2) notification of being placed on a cancellation list. Anyone submitting a form after all Packs have been reserved will be placed on a cancellation list.

Packs must be picked up at OTIS FCU during your designated pickup window. Any Packs not picked up during your designated time may be forfeit and given to the next member on our cancellation list.
