Blue Ribbon Award Nomination Form

Thank you for nominating a UTC or UC Foundation colleague for consideration for the Chancellor's Blue Ribbon Award. 

Nominations must be received by the 10th of every month to be considered for the following month's award. Submissions received after that date will be considered for the next month.

Please remember - nominations should be for only one UTC or UC foundations non-faculty staff member.
