Individual and Family Needs Survey

The Productive Living Board (PLB) uses St. Louis County property tax dollars to help fund services for individuals with intellectual and developmental disabilities (IDD).

We want to hear from people with IDD and their families, even if you don’t use any services right now. Your feedback will help us understand what’s needed and how we can better support the community.

Complete the survey for each individual with IDD. Answer all questions including the demographic information from the perspective of a person with a disability.
