This quiz will review the information covered in the professional development module. The quiz will include true/false and multiple choice questions. To answer the questions, click in the circle beside your response or select true or false from the drop-down menu. 

If your quiz score is 80% or higher, you will meet the requirements to receive PD hours for this module. During the first week of each month, PA NAP SACC project staff will mark attendance and completion for this module. At that time, you will receive an email asking you to complete your evaluation in the PD Registry. After your evaluation is complete, the completed module will appear on your Registry transcript. If you score 80% or higher on the quiz, you will be prompted to close out of the survey.

If you score below 80% on the quiz, you are invited to review the module and take the quiz again. If you would like to do so at a later time, you will be prompted to close out of the survey and return to the module at a date that is convenient for you. You will also have the option to re-take the quiz directly.

If you wish to receive PD hours, please be sure that you have an account in the PA KEYS to Quality PD Registry and include your name, birthdate, your Registry ID number, and your email address at the end of the quiz.

If you do not know your Registry ID number or do not have an account, please open a new tab in your browser and follow the instructions below:

1. Visit the PA Keys website found at www.pakeys.org and click on the square that says PD Registry Login.
2. Click the blue Log in square in the upper right corner and sign in with your email and password, or, if you do not have an account, follow the prompts to create one.
3. After signing in to your account, your Registry ID number will be listed with your Individual Profile information.
