Please fill out the form below to request grant funding for remote working mini-grants arising due to the COVID-19 and tornado crises that hit Middle TN. *Please save a copy of your work in a word document on your desktop, as there have been reports of issues within submitting. We are working with Survey Monkey on this issue.*  

Criteria:
·         Your organization must be a CNM Member, registered as a 501c3.
·         Organizations should have an operational budget size of $2,000,000 or less for the most recently completed fiscal year.
·         Limited to hardware (up to $750 limit for hardware), software and subscriptions to online platforms. We encourage looking at TechSoup for nonprofit discounts.
·         One application per organization. Applications are currently open only to organizations who did not receive Remote Working Mini Grant funding in the first round (Spring & Summer 2020).

Considerations:
·         Funding is first-come, first-served. 
·         We will be sending out checks within one week of approval. Grants will range from $300 - $1500.
·         If you have already purchased these items, please attach receipts.
·         Once you submit the survey, it will redirect you to a screen that says thank you. You will not receive a confirmation email.
