Additional Information:
  • To help ensure a smooth process, please have your student-athletes bring a completed Part 1/Page 1 of the LHSAA physical form (either one-page or two-page version), signed by a parent or guardian.
  • Parents and guardians are welcome to accompany their athletes through the event.
  • In case follow-up care is needed or if you'd like to establish a primary care provider or receive updated vaccinations, our OLOL Health mobile unit will be available for assistance.
