Who is conducting this survey?
‘SUP Medway is a Substance Use Prevention coalition with the mission to work collaboratively to create the conditions where Medway kids & teens thrive and steer clear of addictive substances.
What is the purpose of this survey?
This survey is one of several measures the coalition uses to understand what things contribute to the risk of youth drug use as well as the things that make it less likely for local youth to use drugs. This anonymous information keeps the coalition’s prevention efforts data-driven and focused on effectively addressing the local challenges and reinforcing the protective factors to keep and improve Medway as a healthy and safe community, especially for youth.
Are my responses anonymous?
Yes, this survey is completely voluntary and your answers are anonymous. While your honest responses are very valuable for the purposes of youth drug prevention efforts, you may skip any question you are not comfortable answering. No personally identifiable information is collected with your survey responses. To reassure participants of complete anonymity, if you wish to be in the drawing for a gift card incentive, you will be directed to a SEPARATE form to enter your contact information. There will not be any penalties, charges, or consequences for your honest responses; rather, the goal of this survey is to get population-wide information such as norms, themes, group perceptions, and trends.
