5 to 10 minutes for each Tip Sheet.

We would appreciate your review and comments on these two Family Presence documents that have been designed by, and for, patients and families with IMAGINE CITIZENS Collaborating for Health in partnership with the Canadian Foundation for Healthcare Improvement (CFHI), Patients Canada and the Canadian Patient Safety Institute (CPSI) to help support Family Presence and Participation efforts across Canada. This survey is voluntary and anonymous.

Comments received will be reviewed periodically by IMAGINE CITIZENS’ volunteers so that any significant edits can be incorporated every six months or so.

The purpose of this survey is to understand if and how this material helps patients, family members, advisors, or citizens to begin a positive conversation with healthcare provider(s) about

      • Being a partner in care
      • Influencing family presence and participation policies and practices.

There are two surveys with similar questions for both Tips Sheets.
 
The first survey is about the document, "Partner" Tips, which is intended to be used by patients and families at the point where you receive your healthcare (e.g. at the bedside, in an office or clinic).
 
This survey is about the second document, "Conversation" Tips, intended to be a resource for patients and families in conversations with healthcare providers, or organizations, or communities.
