Dear prospective YAB member, thank you for your interest in being part of our board for the 2026-2027 school year! Being a part of YAB is a big commitment and also a wonderful opportunity for growth, leadership, and service. We are excited to learn more about you and your interest in myHealth.
After receiving your application, current Youth Advisory Board members will review applications and select candidates for interviews (held in person at myHealth for Teens & Young Adults on Saturday, April 18th).
Not all candidates who apply will receive interviews, and not all candidates who are interviewed will be offered a position on the board. You can expect to hear back from us by April 8th if you have been selected for an interview and by the end of April if you have been selected as a YAB member for the 2026-2027 school year.

As you fill out this survey, you may want to type your answers separately and then come back to copy and paste your answers when you have finished them all to prevent losing your work.
For essay style questions, we suggest answers between 100-300 words, although you may write more or less than this. We discourage the use of AI to complete this application and instead want to hear about your own unique thoughts and experiences.
