2026 Community Stakeholder Survey

Valley Medical Center is conducting a Community Health Needs Assessment (CHNA), a comprehensive report that helps us better understand the health needs of the community our health system serves. Valley has engaged an independent third party, Forward Community Health Consulting, for this work.

As part of the assessment process, Valley is asking community stakeholders to add their voices in prioritizing significant health needs occurring in the communities they serve. The information collected with help Valley focus strategies and partnership-building to address these unmet needs.

The health issues listed below have been compiled from publicly available data from trusted county, state, and regional health agencies, ensuring relevance and credibility for Valley’s service area . There is room at the end of the survey to add additional thoughts and feedback on the health issues you see in the community.
