Part 1: Application — Tell us about yourself, your career, and what you hope to gain from the program. Please have your one-page resume and formal headshot ready before you begin; each file must be under 16 MB.

Part 2: Research Survey — A set of questions on women in supply chain and the impact of AI on roles, skills, and career advancement. Your responses will inform research designed to help organizations better develop and advance women in supply chain. Survey answers are not considered in application decisions and will be reported only in aggregate — individual responses are never shared or attributed.
