The Lung Health Foundation (formerly the Ontario Lung Association) is developing new cannabis education resources for healthcare providers and young adults (18-25). We would like to gather your feedback on the format and content of these new resources to ensure we create them in a way that is useful and relevant to your practice.

This survey will take approximately 10 minutes to complete. Your answers will remain confidential and anonymous and will only be seen by the project team. Only summarized de-identified data will be shared outside of the project team. If you have any questions, please contact Sharon Curtis (Project Manager) at scurtis@lunghealth.ca or 647-883-4908.

The survey deadline is: Wednesday, September 30th, 2020.
