The information entered in this form becomes part of the Speaker Agreement and will appear on the conference website. The form requires a headshot and bio file upload. If you will be speaking in multiple sessions you will need to fill out this form for each session.

Before submitting your proposal, please review and sign the speaker agreement linked here. The speaker agreement must be uploaded and submitted as part of your proposal.
