2026 NCFH Scholarship Application
The NCFH Scholarship is open to all employees and board members of a health center interested in pursuing or continuing their career in community health and dedicated to improving access to care for the agricultural worker population. The Scholarship Program is made possible exclusively through funds raised as a result of sales of NCFH Commemorative Artwork. While we encourage individuals who have worked in agriculture to apply, it is not a requirement.
APPLICATION CHECK LIST: Please upload the following items to be considered for the scholarship award.
Applicant signature is required upon submission.
Upload or paste a Personal Statement describing why you choose to work in the community, relevant experiences, achievements, and goals, your commitment to the agricultural worker community and how your current educational pursuits support and enhance your work and career.
Upload a Letter of Support/Approval from your immediate supervisor (required).
Upload a Letter of Reference from an employer, supervisor, mentor, or coworker addressing your contributions to improving access to care for the agricultural worker community and your ability to complete your goals.
Upload your current résumé.
