Study Title: Patient vs. Provider Perspectives on the Use of AI in Orthopedic Practices

- You are being asked to participate in a research study. This consent form contains important information about this study and what to expect if you decide to participate. Please consider the information carefully. Feel free to ask questions before making your decision whether or not to participate.
- Your participation is voluntary. Your participation in this research is voluntary. You may refuse to participate in this study. If you decide to take part in this study, you may leave the survey at any time. No matter what decision you make, there will be no penalty to you or loss of benefits to which you are otherwise entitled.
- Purpose of the research. The purpose of this research is to compare and contrast the varying opinions between patients and providers on the use of artificial intelligence in orthopedic practices.
- Number of people being asked to participate in the research. The target sample size is 385 patients who have had an orthopedic procedure in the past 10 years.
- Duration of your participation. The survey is estimated to take 5-15 minutes to complete.
- Description of procedures to be followed in the research. After providing informed consent, participants will complete a series of multiple-choice questions regarding various statements that pertain to their thoughts or opinions about AI use in orthopedics.
- Reasonably foreseeable risks or discomforts. There is minimal risk in taking this survey.
- Benefits to you or others that may reasonably be expected from the research. We are hopeful the answers gathered from this survey will provide an in-depth evaluation of artificial intelligence in the setting of orthopedic medicine and give insight into the potential impact artificial intelligence may have in the field of orthopedic medicine.
- Alternative procedures. There are no alternative procedures.
- Confidentiality of records identifying you. No identifying information will be collected by researchers.
