2026 Annual Award Nomination Form

Completion Instructions

Thank you for participating in our annual award nomination process. Anyone is welcome to complete a nomination form; however, only submissions that include complete contact information for the submitter will be accepted. You may submit as many nominations as you like, either in a single submission or through additional forms completed at a later time. Submission of a nomination does not guarantee award selection. For the Distinguished Citizen Award, please remember that it is kept a complete secret until the event—your assistance in honoring this request is greatly appreciated. Forms must be submitted by February 5, 2026 to be considered. All submitters will be notified via email regarding the status of their nominations, whether selected or not.

Only businesses or organizations that meet the minimum eligibility requirements will appear in the dropdown menu on the nomination form. For awards open to the public and not exclusive to Chamber member businesses, organizations, or individuals, you are encouraged to provide an open-ended response in the designated text field. This ensures that all eligible nominees, regardless of affiliation, can be considered for these inclusive awards. We value your input and appreciate your thoughtful nominations!

Award categories are assigned to businesses/organizations based on their membership category with the Chamber. Please contact our office with any questions regarding membership categories or if you feel that a business/organization is missing from a dropdown selection.

The award nomination period will close at 11:59 p.m. on February 5, 2026. Once nominations close, business/organizations/individuals who were nominated (with the exception of the Distinguished Citizen) will be asked to formally accept their nomination prior to it moving forward to the selection process. Nominations that are not accepted/claimed in the given timeframe will be considered ineligible.

Awards will be presented at the Annual Meeting & Awards event, which will take place on May 21, 2026.

This award recognizes an organization that provides programs and services that make a profound difference in the Menomonie area and has been a Chamber member for more than 6 months during the award period. This organization embodies a spirit of volunteerism, altruism and respect and demonstrates a collaborative spirit through creative community partnerships. This organization inspires others by example and has a proven contribution to enhancing the community’s overall quality of life.

This award recognizes an individual, family or business that has made notable contributions to the agriculture industry and their local communities. Nomination considerations include length of service in the agriculture industry, scope of operation (animal production, crop management and/or other forms of agri-business including agricultural support services), responsible land management and conservation efforts Industry involvement and/or agricultural promotional activities, agricultural advocacy and any industry-specific achievements or recognitions.

This award recognizes an individual who is an emerging leader in the Menomonie Area, regardless of age. To be eligible, nominees must be employed by a Chamber investor business in good standing and have less than five years in their current career position. They should demonstrate leadership ability among peers, excel in their work, and show potential for career advancement. Additionally, nominees must have made a meaningful contribution to their profession beyond their regular job duties. Nominations are valid only for the year submitted, though individuals can be nominated annually if not selected for an award.
