This survey asks about concerns that you or someone you care for have had with their healthcare, and your experience of giving feedback to services.
'Concerns' mean things like treatments or procedures that went wrong, missed or delayed diagnosis or treatment, or medicine errors, but there may be other examples where things went wrong. Remembering these might bring up some feelings for you and we provide contact details for organisations that might be helpful at the end of this survey.
You might have more than one care concern but for this survey we are interested in your most recent concern. The goal is to understand how people who have healthcare concerns deal with them and their experience of using feedback services like PALS or Friends & Family Test.
