Introduction
Thank you for your interest in taking part. Within this survey we will be focusing on your experience of using community eye care services such as opticians. We will also explore your views on some of the changes proposed in the recently published NHS 10-year plan, which proposed delivering more eye care services in community settings, such as opticians.
Answers are optional, and you can choose to skip questions or exit the survey if you want to. You can take as long as you need to complete. There are some comment boxes throughout the survey. A small selection of comments may be used anonymously in our future communications.
As this research is in response to NHS England’s 10-year plan, some sections will only be seen by those living in England. We are interested in hearing from both people with sight loss and from those who do not have a sight loss diagnosis but use community eye care services, especially opticians.
Please remember your participation is entirely voluntary, so you can exit the survey at any point.
