State Health Assessment - Community Health Survey

The purpose of this survey is to gather opinions about health problems in West Virginia.  The West Virginia Department of Health and Human Resources (DHHR), Bureau for Public Health will use the results of this survey and other information to identify the most pressing health problems. Your responses will help state and local health groups focus their efforts on improving health and wellness for all.
