Thank you for taking the time to complete this survey. The purpose of this survey is to gather information from community members on health needs, concerns, and priorities. The survey also asks about your health and general demographic information. Your responses will inform a Tribal Health Assessment and guide future planning, programs, and services to improve community members' health and well-being.
This survey should take about 30-60 minutes.
Your participation in this survey is completely voluntary. Some questions may make you feel uncomfortable or distressed. You may choose to skip any question or stop taking the survey at any time. There are no penalties or loss of benefits if you decide not to participate.
The first 4,000 people who complete the survey will receive a $10 gift card. Everyone who takes the survey will also be entered into a drawing to win a $500 gift card. We will give away one $500 gift card for every 1,000 survey responses we receive, with up to 20 gift cards available. To receive the gift card, you must have an email address. You will provide your contact information, including email address, after the survey is complete.
All responses will be kept private and reported only in summary form. The survey does not ask for your name or other information that could be used to identify you. Your name will not be included on any reports that come from this survey. Data will be stored securely and accessed only by authorized staff involved in the Tribal Health Assessment.
