CFP needs your input regarding health insurance costs for 2026. Due to the likely expiration of the enhanced premium tax credits, we are seeking feedback from our patients in order to provide affordable options. This survey will close on 12/15/25.

.What is your current insurance status?
.What will be your insurance status for 2026?
.If you currently get insurance through the ACA Exchange "marketplace", what is your expected monthly cost increase per person next year (after accounting for the expiration of enhanced tax credit)?
.If you noted that you expect an increase in costs for next year, how do you plan to address this?
.For the last question, if you selected "Switch to a high deductible plan that doesn't cover any visits" or "Go without insurance", are you interested in a Direct Primary Care payment model? Direct Primary Care is a healthcare model where patients pay a flat monthly or annual fee to their doctor, without using insurance. Monthly fees are estimated at $100-150/month per person (additional discounts for family memberships). All visit types are included. Labs, Testing, and Procedures may come at an additional but discounted cost.
.If you answered "Switch to a high deductible plan that doesn't cover any visits" or "Go without insurance" in response to Question 4 and had to choose an option below now, which option would be preferable for your family's needs?
