This survey intends to get to know the needs and experiences of the South Phoenix community. We, the Cihuapactli Collective, are working to bring a Wellness Center and Food Forest to South Phoenix and would like your input to guide our services. It takes about 5-10 minutes to complete.
This survey is anonymous, no personally identifying information is collected unless voluntarily provided.
Note: The Fry's gift card is only for South Phoenix residents that complete the survey. The first 300 people will get a $20 gift card and two people will be entered to win a $500 gift card.
