Volunteer and Educational Booth Registration

Thank you for participating in our Volunteer and Interactive Education Booth registration.

Please submit ONE entry for each person that will be volunteering. You only need to include booth details on one of the entries. Completing this registration will speed up your check-in time on the day of the fair!

All booths are required to stay until 12:00pm. The fair will begin at 8am and volunteer registration will open at 7am.

Remember, the purpose of the Community Health Fair is to educate and promote health awareness to participants.
No organization or individual is permitted to sell goods/services at a booth. Distribution of business cards is acceptable.
Each submission will be reviewed for appropriateness by the organizing committee. If follow-up is needed we will reach out via email.
